Visits
Before Your First Visit
Prior to your scheduled visit we will send you a packet containing, among
other forms, a comprehensive health history questionnaire. The questionnaire
is 11 pages and we ask patients to allocate one hour to thoroughly complete
this form. The information you provide helps Dr. Hirani properly diagnose
your condition(s). The questionnaire also acts as a skeleton for the detailed
history that she will obtain from you during your initial visit. The packet
will also contain medical release forms that you may send to your other
care providers that will request them to send copies of your previous
labs and histories to us. For your convenience, you will also find a map
and directions to our office.
Your Initial Consultation
We ask adult patients to budget 2 hours in the office for their initial
consultation. Most of this time is spent with Dr. Hirani during which
time she will obtain your medical history, perform a complete physical
exam, provide nutritional and dietary recommendations, recommend laboratory
testing, and provide you with the outline of a health plan. We allocate
about 30 minutes of this time to check you in, collect any specimens for
lab work, and to check you out.
Please remember to bring with you:
- Your completed health history questionnaire. Having your packet completed
maximizes your time with Dr. Hirani.
- Your insurance card
- Any recent relevant lab results, dexascans, etc.
Subsequent Followup Visits
Your first followup visit will usually last one hour. During that time
Dr. Hirani will review your progress to date on your plan, discuss lab
results, and update your plan depending on your lab results and your progress
to date. Subsequent office visits vary in time from one-half hour to one
hour depending on your progress and success to date. Established patients
who are now doing well sometimes arrange for 15 minute phone consults
to update their plan with Dr. Hirani.
