Office Visits
Before Your First Visit
Prior to your scheduled visit we ask you to download the comprehensive health history questionnaire. The questionnaire is 11 pages and we ask patients to allocate one hour to thoroughly complete this form. The information you provide helps Dr. Hirani properly diagnose your condition(s). The questionnaire also acts as a skeleton for the detailed history that she will obtain from you during your initial visit. You may wish contact your other care providers to request copies of labwork or imaging you wish Dr. Hirani to review.
Your Initial Consultation
During your initial consultation Dr. Hirani will obtain your medical history, perform a complete physical exam, provide nutritional and dietary recommendations, recommend laboratory testing, and provide you with the outline of a health plan. Also included is lean muscle mass testing to determine caloric intake. After your consultation time with the doctor our staff may collect blood for any testing that is recommended.
Please remember to bring with you:
- Your completed health history questionnaire. Having your packet completed maximizes your time with Dr. Hirani.
- Your insurance card that we forward to outside labs
- Any recent relevant lab results, dexascans, etc.
Included in the Initial Consultation
- 90 minutes of un-interrupted time with Dr. Hirani. (60 minutes for abridged new consults and children)
- Detailed review of medical history and current symptoms.
- Physical Exam by Dr. Hirani.
- Lean Muscle/Body Fat Testing via bio-impedance analysis for calculation of basal metabolic rate (BMR) to determine protein intake.
- Nutritional and Diet plans.
- Specialized and specific laboratory recommendations individual to your condition. See Bloodwork page.
- Recorded Audio CD of plan and recommendations.
- Typed instructions for plan
- Typed action list of supplement regimen with descriptions and dosing of each.
- Dr. Hirani does not like to keep her patients waiting. We do not double book her appointment slots.
Subsequent Followup Visits
Your first followup visit will usually last one hour. During that time Dr. Hirani will review your progress to date on your plan, discuss lab results, and update your plan depending on your lab results and your progress to date. She may write prescriptions at this time. Subsequent office visits vary in time from one-half hour to one hour depending on your progress and success to date. Established patients who are now doing well sometimes arrange for 15 minute phone consults to update their plan with Dr. Hirani.
